If you are consenting for your minor child to get a tattoo or piercing, please print the appropriate form, have it signed and notarized. If you are having it notarized in the shop please bring parent/legal guardians ID and copy of the minors birth certificate. Please bring it with you to the appointment, or when you come to the shop.
